I have read somewhere on VJ web site but just simply cannot locate where I read it at.

I read to request a copy of your medical so you can bring it with you to the U.S.

(this is not the CD you get)

It stated that you can request a copy and may or maynot get it.

That the copy will help during the AOS process.

If anyone can point me in the right direction that would be great.

Her medial is about 4 hours.

They will give you a CD and a paper that indicates the vaccine (3025) you have received after the whole medical process. The paper is what you need for the AOS

Never read anything about getting a copy of the medical, so not sure where you read on that one.

I know you don't need a copy of the medical for AOS , you do need to take the 3025 to a civil surgeon and get it transcribed to the I-693.

You can get multiple copies of the DS-3025, I think you get two by default.

Hank, you do need the DS-3025 for AOS. If you aren't missing any vaccinations (or even if you are, actually), you can simply submit the DS-3025 and never get an I-693, provided you apply for AOS within one year of the medical. Paying for a DS is only required if you are missing something on your DS-3025, and not something I recommend unless you just WANT to pay the extra money.

Not so. There have been two just recently from the Philippines forum (sure there are many more on VJ as a whole) that got AN RFE for sending in the 3025 and not the I-693. I know what the I-485 instructions state, but I also know USCIS will issue an RFE to get the I-693 many times (there have been a few that slide through with the 3025, but they seem to be the exception). To get the 3025 transcribed to the I-693 was $40 for us.

I have read many times here on VJ over the years of issues regarding the 3025 and I-693 for AOS.

I will just add this as a "side note." If you are concerned about your gal getting the needed immunizations/vaccines, when she gets here (USA) take her to the local county health department and they will make sure she gets all the shots she needs and at a much cheaper price that what the civil surgeon (CS) will charge. This is what my gal and I did. And, with the paperwork from the health department and the I-693 it was taken care of by the CS front office at "no charge." Yes, I couldn't believe it either... I did try and offer to pay but the lady would not take any money. Best of luck!

The K-1/2 beneficiary will be required by the embassy to have a "medical" done prior to the visa interview. Part of this interview will require that previous vaccinations are documented and potentially additional required vaccinations performed. Once this is complete the doctor will note this on a DS-3025 form that will be sent to the embassy. During the medical appointment request an official copy of the vaccination records from the physician (or a copy of the completed DS-3025 if they will give it to you) as this will be helpful to have on hand in the future (specifically it can be helpful when filing for Adjustment of Status if your DS-3025 form is not available).

The medical report and visa photographs are submitted by St. Luke's Medical Center Extension Clinic directly to the U.S. Embassy, with few exceptions. In most cases, only the passport, CD of Chest X-ray, and Vaccination Documentation Worksheet are given to the applicants.

I think I am covered. I advised Bebe to ask for a copy of it when they giver her the CD.

I haven't looked on the CD lately, but I think it might be on there, too. Or maybe it was the 3025 itself.

According to Hank, some have had issues with using the 3025 for AOS, but we were approved without using a CS, but that was over a year ago. I know of one here who got an RFE, but her 3025 was incomplete.

The statement just says that you will be given the vaccination record (DS-3025) with the CD. Once she finishes the medical process she should make sure to have a paper (DS-3025) and a CD in her hands. They say that it is a copy because the original would probably be with the SLEC computer system and a copy would also be in the visa packet sent to the embassy. She should ask a copy of the DS-3025 if upon finishing she is not given it and no need to pay for the said copy. So just remind her a CD and a vaccination paper is all she needs if she has completed the whole medical process.

I have never looked to see what is on the CD, others have said it is just the x-ray images.

I can tell you by experience by not getting the medical forms is a real pain if you are trying to enroll the kids into school.

I had to get 3 school physicals and two need to be treated for latent TB. Luckily, I have experience reading the skin test and adverted a near panic at the school.

The form they got from St Luke's stated she tested positive but since we had no medical report she was treated as though she had td. Well, the State Health department decided to play it safe and now she will have to get treated for the next 9 months. They said if they had the notes from the medical she probably would not have to get treated. Go figure.......
