Just wondering what are the options if the original sealed I-693 is lost? Can the doctors office provide another? Is a copy of any use?Would a whole new exam be required?

You can go back to the issuing CS and get another copy--hopefully--or redo the medical. Either way, you need a completed I-693 for the AOS if the vaccation records were incompleted during the interview process.

The surgeon's office has your record on file. They might ask you to get flu shot just to extract money from you but they will definitely give you another seal envelope with I.693 form in it. Good luck
