I got exactly the same letter from Raleigh Field Office.

It's not worth arguing about, just get to a civil surgeon and get it done.

My nearest civil surgeon was only 5 mins from home, $50 just for the first visit, they wanted $390 for the TB and syphillis tests (I had no evidence to produce as its all in the brown envelope) and MMR (2nd dose that I didnt get in London) and correct Tdap (apparently was given the wrong one....#######???) then another $100 for the transcription to I-693.

The nice nurse, who was very very knowledgeable about the whole process btw, must have felt sorry for me, told me I was free to shop around or try the County public health centre get it all done there "cheaper" and come back with the results. Of course they were only open til 4 each day so had to leave work early several times over a 2 week period, work were not happy at all, many grumbles from the boss but tough ###### I was going anyway.

So only cost me $13 for the TB test, the rest was free....result lol

Back to the civil surgeon office with the results, 2 days later went back again to sign the forms and they gave me the sealed envelope, for $100 of course, to send off to USCIS via USPS.

About 2 weeks later got the NOA that I was approved and have just heard the card is in the post with no interview.

OK so I don't know where in NC you are, that doesn't matter if you are just seeing the civil surgeon though.

Just to transcribe to the I-693 was $150. ($50 at the first visit and $100 when you sign the I-693 and collect the sealed envelope)

You won't actually see him if you're not having a medical, just the nurse.

I had a 3rd visit to drop off my test results and updated shot records in between.

I'm in Fayetteville, Cumberland County.

The civil surgeon I used was at the Fayetteville Medical Clinic P.C. Suite C

Just had a thought, they made me have the TB test and syphillis test as well as the shots as I had no medical proof of anything to show them other than the DS-3025.

They wanted - $35 for the PPD test, $120 for the RPR test, $150 for the MMR shot and $85 for the TDAP.

It might be worth preparing yourself for that bombshell and finding out where your County Public Health Centre is just in case.

Wow, these USCIS officers in NC, huh? My husband and I are in the AOS process from K-1, as well. We're going through the Charlotte office, and we just got the same thing. No RFE, just straight to NOID. It sounds pretty straight-forward, though, to resolve. Best of luck to you two!

Hello :)

I thought I would give a little update... We had our appointment with the civil surgeon today - Elizabeth Neel, Doctors Express Urgent Care in Cary (101 Maynard Crossing Court, Cary, NC 27513; Tel (919) 651-9867), everyone at the surgery was so nice and helpful!!! Most importantly - they were very knowledgeable about the requirements for Form I-693. I gave them my passport and combo card (for work authorisation/advanced parole) as ID. The combo card was needed as it provided evidence of my married name. I forget to take my Social Security No, which they did ask for, but they processed me without it (thankfully!). I was asked to complete the usual registration forms for a new doctor, as well as Form I-693. I had taken along my own copy of Form I-693 with my details typed into Part 1, which they used. On completion of the required paperwork, the lady at the desk asked me for any evidences related to my vaccination history - I provided the vaccination record I received from the civil surgeon in London, a letter from my GP in the UK with a summary of my vaccinations from birth (I obtained this for the purpose of my K1 medical in London) - as well as a note from the surgery in London confirming the vaccinations I had during my medical - my second MMR and Td/Tdap. My husband and I sat in the reception area as they processed all of my information information, both of us expected to get called in to see the doctor once everything had been processed, but we were called to the desk and asked to pay a consultation fee of $65 - and were then handed our completed and sealed letter for USCIS!!!! It turned out that the lady at the desk we had been dealing with was our civil surgeon!! I had expected a more onerous process and asked her if everything was complete - she smiled and told us that everything was complete and that we had more than enough evidence!!! We both praised God for his goodness in it being so straightforward. I was concerned that I would need more vaccinations...and that as they had quoted over the phone it could cost up to $300...

I now aim to send the sealed letter as signed for recorded delivery, and will include a cover letter and a copy of the Intent to Deny letter... Thanks again everyone for your kind assistance!! I thought the above summary would be useful, as it transpires that a few more North Carolina filers on here have received the same Intent to Deny letter... All the best to everyone!!!!
