So I was on the embassy website and I see now that they make both parties get a medical exam at a government hospital BEFORE the marriage. Is this something new??? It looks like it's required by Egypt, not the USA. If this is correct, how hard is it to get an appointment and how much does it cost? This is crazy!!

I have not done this myself, but I've been told Egypt started requiring medical certificates sometime in 2011. They have to be done at a government hospital. You could try asking some of the hospitals listed here for more specifics: http://www.egypt.alloexpat.com/egypt_information/government_hospitals_in_egypt.php?

If the exam is anything like the one I had to do for my insurance, they will basically just make sure that all of your limbs are attached and then sign a paper for you. Definitely bring photocopies of your passport, and multiple passport photos, which you need for pretty much everything here.
