DS-156 Middle / Patronymic name solution

I have searched all over for an answer to a frequent question and not found one.

In some countries the women do not have middle names. Many of us use the Patronymic name as the middle name on all the paperwork

Within the intl. passport the Patronymic name is listed in the native alphabet but not in English and the DS-156 asks for the middle name 'as it is written' in the intl. passport.

I have asked UStraveldocs about this and they have replied that you put only the first and last names into the 156 in this case. You may then place the Patronymic name in the box for other middle names if you wish. By saying 'if you wish' I gather that even that much is not required.

In Ukraine and Russia, the patrynomic IS the "middle name" For Alla we printed her Patrynomic as her middle name in English (transliterated.)

This is simply a non-issue for Kiev. I assure you, they know all about transliterations, various spellings, patrynomics and everything else. Alla's middle name does not appear on her visa. They will accept any reasonable spelling of her patrynomic name on the DS156. Alla does translations for the consulate (or did when they required it) and this is simply not an issue.

When she changed her name upon citizenship she kept her "middle name", patrynomic, the same and only changed the family name.

This is semantics and nothing more. No worries.
