No DS-3025 for i-693a? Best advice please!

Hi again all. My fiance, oops, wife got a sealed envelope from her examining doc in Moscow prior to her interview. She passed this sealed envelope to the embassy and of course had another, bigger sealed envelope that she relinquished at customs. At no point did she actually see her ds-3025. She has her original imminuzation records in Russian, but that's about it, and she doesn't think anyone without a Russian medical background could decipher it. She really really doesn't want to do (or redo) any vaccinations unless 100% needed.

So I guess the questions are:

1) where is our DS-3025 now and how could we request a copy?

2) Aside from sending in the AOS packet without the i693a supplement and playing RFE roulette, what are our other options?

3) Has someone had experience stateside with either a specialized translation agency or bilingual doctor to get the supplement filled out and/or records translated?

Thanks in advance!

Is there anyway of having the 3025 faxed over from the Moscow doctor? They should have a copy/record of it.

If not, she could have the blood tests to check for antibodies and if none, get the vaccinations. The 693A is only for vaccinations so she wouldn't need anything else.

As for the translation of her vacs record, is there any online translating websites? It's a long shot but if there is, maybe printouts of translations could help. Is there a major university near you? Sometimes the language department can help/translate. Or even community organizations that like a Russian/American club? Do they exist?

Hope this helps!

We also had no DS-3025. We simply took my husband's original vaccination records to the civil surgeon. She had plenty of documentation to help her translate it. She was able to fill out the I-693 without the DS-3025, as I learned here months ago many civil surgeons will do. Just try to find a nice and reasonable one. And be sure to mention when you make the appointment that you do NOT need the full medical.. It's always better to say this in advance. Good Luck!
