Brazilian phone number on I-129F

Hello, I'm filling out the I-129F... My fiance is brazilian and his phone number is 8 digits long plus a 2 digit area code, and there is a country code of 55... On the I-129F the phone number spot is set up like a US phone number with a 3 digit area code, like (_ _ _) _ _ _-_ _ _ _ and asks for an extension... How would I write in a brazilian phone number on this??? Would I just write in the 8 digits and 2 digit are code and ignore the structure of the answer box?

And in the address should I put the CEP in the postal code box, the zip code box, or both?
