Hello, I have a question,

When k-1 visa holder is applying for application of status does the $1070 include fee for application of employment authorization and advanced parole when filed all together? Also, how does one know where to get a surgeon to sign medical form? Also can the intending immigrant send application without ssn?

The filing fee for an Employment Authorization Document and for Advanced Parole is waived when they are filed together with an I-485.

Also, if you have trouble using the USCIS list to find a civil surgeon that will do what you want (like I did), you may find one in this thread - just search for your city:

I called several CSs listed on the USCIS list for my city and they all told me, "We don't do that" when I told them what I needed. So I used the above VJ thread to find a "real" civil surgeon in my area that would complete the forms. I don't know why the others were listed on USCIS's list - they had no idea what to do with immigration paperwork.
