Hello I was filling up my N400 form and I need help with schools and employers:

Some schools or employers I don't really know their exact address, can I put the city and the state? what about the dates, if I don't remember them exactly can I put month and year instead of month/day/ year?

Try to get as much information as possible from your previous tax returns about the addresses of your employers. If you don't know the exact dates, you may be able to put approximate dates but may want to write for example "Approximately June, 2010".

The last name is what's he rcurrent legal last name meaning if her names was changed legally after marriage, you should use that. Her married last name will be different from her last name o nthe birth cert and that's ok.

I agree with using your wife's current legal name on the form. As for the addresses and dates. I believe it's ok to use the month and year if you can't remember the exact date, but for the addresses, how about trying a google search on the names of the schools and businesses. Usually they have a contact page on their website that has the official address.

Thank you Jamericanlove, That's what I've done as well I put the approximate date (month and year) for my wife's last name I did put my last name since she changed it to mine when we got married. Thank you so much again.
