DS-156 (Questions 30 & 31) /B2 Visa or not?

Hey there community,

Unfortunately I've got myself stuck again. I did some deep VJ research and couldn't find a definite answer on this, so any help here would be GREATLY appreciated. Last November I was denied access into the United States while trying to cross from Canada. (Full story >> http://www.visajourney.com/forums/topic/402026-biggest-mistake-of-my-life/ )

I've never had to do anything super special when crossing the border because Canada has some sort of agreement with the U.S (Is this called VWP?).

Anways, when looking through my passport, I came across multiple stamps when entering the United States. Some of them had nothing written, just the stamp-- but 2 of them had B2 written on the little line.

So I guess my question is this:

1 - For question 30 on the DS-156, do I select "Yes" and put B2 Visa? And, are the rest of my stamps that look like this (without B2 being written) also considered to be B2 Visa's?

2 - For question 31 on the DS-156, do I select "Yes" for being denied a visa (under Canada's special agreement with the U.S), or was I just simply denied entry?

Once again, any help given is GREATLY appreciated. We just received our NOA2 about on September 2nd, and apparently it's been valid since July 12th.. so we are on an EXTREMELY tight schedule with with the amount of time given to us..
