Hello. I am filling up the CFO form. May I ask some help what to write in the field "Address & Contact Numbers in the Philippines"? I am renting an apartment and I will be leaving the country on September 12. Should I just write my parents home address and contact numbers? Salamat po...

I wrote my current address and contact number on my personal info portion (although I know I will be leaving soon)... I wrote my mother's address and number on a portion that says nearest relative part...

One section is your address, the other should be your parents info. good.gif
