Consent for Minors to travel - form question for their Dad's consent

Yeah, we don't have problem with DSWD & Immigration in the Philippines. But when my fiance called to US Embassy a month ago, the Consent to Travel from Other Parent is needed and better to prepare during the interview. Our laywer is asking if there's any standard format from US Embassy... which we are not sure. She was trying to call the US Embassy in the Philippines but no luck to talk someone yet...

Of all the Filipina I have read of here on VJ getting visas with their children never have I a read of a single one needing a consent to travel form at the embassy. Not one.

just an update guys! My fiancee emailed USEM about the above subject and below is their reply :)

"You may submit a signed notarized letter from the other parent allowing the child to travel. The adjudicating consular officer determines if additional document is necessary as the application is processed."

Key would in that email from USEM... MAY= as in not required and not asked for. I have never read of the embassy ever asking for a letter from the other parent... not once here on VJ.

TRAVEL CLEARANCE FOR MINORS

A travel clearance is a document issued by the DSWD to a Filipino child (below 18 years of age) traveling abroad alone or with someone other than his/her parents.

WHO NEEDS A TRAVEL CLEARANCE? A minor traveling alone to a foreign country ; A minor traveling to a foreign country accompanied by a person other than his or her parents.

WHO DOES NOT NEED TRAVEL CLEARANCE? All minors other than those cited above, for example: A minor traveling to a foreign country with either parent or with his or her solo parent or legal guardian; A minor traveling abroad whose parents are in the Foreign Service or living abroad or are immigrants, provided he/she is holding a valid pass such as a dependents visa/pass/identification card or permanent resident visa/pass/identification card which serves as proof that he/she is living with parents abroad and their travel does not constitute child trafficking. - See more at: http://www.dswd.gov.ph/?s=travel+clearance+children&.x=0&.y=0#sthash.gLaNkCCz.dpuf
