I've read the link you gave and i finally knew that the form was not properly completed on the MMR part "Insufficient Time Interval" and some vaccines (Hib & Meningococcal) should be marked as "Not Age Appropriate", etc.

Should i let the embassy know about this so i can contact the appointed clinic to ask them to fix it?

I think it is too late to fix. You are done with the embassy and visa. Your medical records from Indonesia are final and are in the sealed envelope that came with your visa. And the doctor there thinks he is doing them correctly and may not be willing to change anything. Plus the embassy would have to agree to resealed your records. I doubt you can pull that off.

You can see now why people get RFEs. It is not because USCIS refuses to consider a DS-3025. It is probably because it wasn't filled out properly.

You still have chance that USCIS would accept it just because it is marked complete. Your other questions about a civil surgeon. You can show him any shot records you have. It can be from you're own doctor in Indonesia, the DS-3025, or a health clinic in the US. His job is to look at all the records and put them onto one approved form...the I-693. You might call around and see if you find one who knows the K1 process and will do only the vaccination part and charge you $25-35. That would be a reasonable fee. But if you can't find one then maybe think about trying your DS-3025 before you spend a lot of money or have a new exam.

I agree with Nich-Nick. It isn't properly filled out, but you do have all the required vaccinations. So, try sending a copy of the DS-3025 and see if it is approved. If you get an RFE, then worry about finding a civil surgeon and getting the I-693. It won't delay your case too long if you do get an RFE for it, and if you don't, then it works out well.

There is no check mark at all on her DS-3025 for the Rotavirus. I know that is not age appropriate, will that be a problem because there is no check mark anywhere on the rotavirus line????

Nobody can tell you for sure, they might just approve it, or they might send an RFE because of it. If they send an RFE, you'll have to go to a civil surgeon to fill out I-693.

If all the other check marks are there, maybe they won't notice just one missing.

I am hoping that they are smart enough to realize it's not age appropriate since the Rotavirus immunization is for infants and this will be AOS for a Spouse. LOL

Hi everyone,

I just talked to my friend (she's also an Indonesian), she filed for the AOS two months ago, she said that the clinic didn't even gave her the copy of DS-3025, they gave only vaccines card. She copied the card and sent it along with the AOS packet and didn't get any RFE for the medical but she got RFE for the I-485 thingy.

So, hopefully (and Pray hard) i will not get any RFE about the medical since i have the DS-3025 form, vaccines card, and also the certified letter about vaccines i've received. I'll make copies and send it all along with the AOS packet.

My fiancee had the "not age appropriate" box checked for the flu vaccine instead of the "not flu season" box. Don't know how it's not age appropriate, hopefully it won't be a problem. The medical was done not in the flu season, but we'll be sending in AOS during the flu season.

Everything else seems to be fine on her form though. Hopefully they won't send an RFE for it.

Just a comment on that. If you are cleared of everything on a properly filled out DS-2025, then you are done. A waiver for not flu season means you don't have to get the shot, even if it is flu season when your case gets evaluated for AOS. they go by the date of the last medical exam.

That is also another example of a DS-3025 that was not filled out correctly and could be rejected. The flu shot is age appropriate for everyone older than 6 months. Sounds like your panel physician did not pay attention to the November 2010 update. Prior to that you had to be 50 or older for a flu shot.

Sounds like your panel physician did not pay attention to the November 2010 update. Prior to that you had to be 50 or older for a flu shot.

That's what I thought. Seemed kinda weird for a flu shot to not be age appropriate. The "not flu season" box is the one that should have been checked. Oh well, it's too late now, just gonna send it in as it is and hopefully they'll see that it wasn't flu season and let it pass (or be stupid and just see some checkbox on the line and let it pass).

We did DS-3025 only. That being said, her shots were complete, except for the age-specific ones. If you need booosters or further shots, I think you will need to get a CS involved and have them furnish a SEALED I-693 for USCIS. And yes, take the DS-3025 to the CS when you go. It has to be a CS, not "any" doctor.
