1) if needed handwrite it in. The other option is an attachment sheet. For a name, I would handwrite it in and use an attachment sheet for addresses etc.

2) you can fax it to her, but she will have to mail it back. It should be an original signature.

3) the photo should be on photo paper, so it should be a photo... however there is a tool on the National Visa Center website, that she can use to do a picture herself. There is also http://www.epassportphoto.com/. The copy of her final divorce decree can be a copy. The original will have to be sent to the NVC whenever the petition is approved.

I have another possibly dumb question: On i 130 form Question C1 it asks for Family name of relative (in my case my wife) Do i fill out my last name OR are they asking for my wife's maiden name? A bit confused because underneath in question 7 it asks for "other names used (including maiden name).

If i'm understanding correctly then I am filling out her married name (my last name) and question 7 is her full name before we were married?

Sorry for the nagging.

If your wife has taken your surname as her surname, you can put your surname in the "family name" spot. If your wife still uses her maiden name, then put her maiden name in the "family name" spot and leave the "other names" blank or with dashes --- or ----none----.

It all depends on if your wife is planning to, or is using your surname as her surname. :)
