Need Help ASAP

It's been a while since I've been here.

I had put in for my adjustment of statues a while back, and had recieved a permission to work card from the government. However, we recently got a letter saying that they did not have records of my vacinations (which we did send. We're guessing they lost them). They also wanted another form (which is tied to it) sent also by a certain date, or they will reject my application.

Now, the problem is, we've misplaced the letter, and we can't remember the form we need, or the address to send it to. If anyone knows, please let us know as soon as possible. We don't want to have to go through all this again, especially after so long.

Do not know the address to send the RFE response to, but know you need to see a civil surgeon for the I-693.

You may want to call the USCIS and get a tier 2 IO on the line to ask about your case status. Let them know you lost the RFE letter and see if they can tell you what to do. You are only given a certain amount of time to respond to them.
