DFA Childs Passport

Timeline

My fiancee wants to try for the passport for her infant child . She does not have her own NSO birth certificate any longer . The DFA had taken it when she applied for her passport . She has one that she is waiting for , but wants to try to get the passport for the baby today . I read the DFA requirements , but found them a bit confusing on the children . The baby will be traveling with us to the USA . She is a single mother so the child is illegitimate .

For Minor Applicants (below 18 years old) General Requirements:
  • Confirmed appointment (except for 1 year old and below) - OK
  • Personal appearance of the minor applicant - OK
  • Personal appearance of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child) -OK
  • Original Birth Certificate of minor in Security Paper issued by NSO or Certified True Copy of Birth Certificate issued by the Local Civil Registrar and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. Report of Birth duly authenticated by NSO is required if minor was born abroad. OK
  • Document of identity with photo, if minor is 8-17 years old (for first time and renewal applicant) such as School ID or Form 137 with readable dry seal - NOT NEEDED
    • For minor applicants who never attended school, a Notarized Affidavit of Explanation executed by either parent (if minor is a legitimate child) / by mother (if minor is an illegitimate child) detailing the reasons why the child is not in school, is required SHE IS ONLY 1 MONTH OLD THIS IS NOT NEEDED RIGHT ?
    • Marriage Certificate of minor’s parents duly authenticated by NSO - SINGLE MOTHER
    • Notarized Affidavit of Support and Consent to travel from either parent (if minor is a legitimate child) / from mother (if minor is an illegitimate child) THIS IS NEEDED IF THE PARENT WILL TRAVEL WITH THE CHILD ?
    • Original and photocopy of valid passport of the person traveling with the minor OK
    • Photocopy of valid passport of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child) or identification documents (Please refer to List of Acceptable IDs)

She used her SSS ID and Phil Health ID and NSO Birth Certificate Police and NBI as ID and supporting documents to get her passport . The DFA took her original NSO and she will not have that when going for the baby's passport . Will this be a problem with getting the baby's passport ? Of course she has her passport to show as well . She is getting another NSO but it is not ready yet .

So the only 2 things I am confused on if she needs her NSO and the affidavit of consent to travel from a parent . I did some research about the consent on here mostly and found that it is not needed when the parent is traveling with the child . Just wanted to double check since she is going today .
