Confused about I-693, Urgent, trying to mail of AOS today

My wife had her medical evaluation in Spain in April before her interview for K-1 visa. She had the X-rays done in a separate place than the medical evaluation itself (as per the rules in Spain). She wasn't sure of what vaccinations she had, and the doctor told her not to worry, that they can do them in the United States.

During the interview, they removed a piece of paper(s?) from the sealed plastic bag containing the X-rays and medical evaluation, and gave her the resealed bag of X-rays (Im guessing they took out the doctor's form from the medical evaluation) and told her to take it with her in her carry on baggage. The embassy later sent her passport and sealed packet to be turned in at POE.

At the POE (JFK in NYC), the took said packet, but told her that they didn't need the X-rays.

So does she need to get her vaccinations before we send off the I-485 packet or after or what?

If the doctor said "you can get them done in the US" then I think it's a sign that she needs to get vaccinated and get it updated on a I-693. What does her DS-3025 say? (I think that is the form number for the vaccines).

She should have a copy of her vaccination sheet. Does she have that?

I have no idea about this.

She had her medical exam less than 4 months ago, and now needs another one? I call one of the listed civil surgeons here and the said 'they do the I-693 for immigration, the TB test etc, but not vaccinations". Does this mean we have to go somewhere like the health department, get the vaccinations, then get ANOTHER evaluation with the people I just talked to before we send in the I-485 packet?

I don't know where she'd have gotten the DS-3025 form anyway. Everything she was given was sealed and not to be opened.

If I print out a DS-3025 and take it to the health department for her to get her vaccinations, have them sign it, could i send that in in lou of the I-693 since she's had a full medical evaluation less than a year ago?

I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

this is from the AOS guide and it seems unclear to me. The place I called does the I-693 for people, but not vaccinations...and vaccinations are what she needs. Really confused here...

You will need to bring proof that the missing vaccinations were completed, and provide this to the civil surgeon (the shots don't need to be done by a civil surgeon). The civil surgeon should only complete/transcribe the immunization section of the I-693 (another physical exam is not required assuming the last one was completed recently).

I would call the Spanish clinic and see if they could scand and email you a copy of that sheet.

There are a lot of vaccinations she would have to take again, basically all (or almost all) she has ever gotten from birth.

They don't give you all the shots you need from birth. They go by your current age group and other things, not a total do-over.

It sounds like her vaccines weren't complete, though OP, in which case you need vaccines and vaccine transcriptions done at a Civil Surgeon (and they give you the I-693). The transcription should cost like less than $40 and you pay for the vaccines.

First follow up on the DS-3025, though. And no you don't print it out... you need a copy of the one they filled out in Spain. Do that first and then worry about the next step.

Be aware, though, that many civil surgeons won't do just the vaccine form--they'll insist on re-doing the whole physical. There's a thread here on "civil surgeons who won't rip you off" that may be useful for your situation.

we went today to the health department (a doctor there was listed as an approved doctor on the USCIS website) and he gave her the immunizations and filled out and signed pages 1 and 5 of Form I-693 and sealed it in an envelope with scotch tape and put his initials on the tape. He's their doctor for immigration purposes and seemed to know everything about the I-693 and the process in general.

I guess I just turn in that envelope in place of a full I-693 with a note attached saying this was supplementary to the medical eval done in Madrid? The instructions on page 5 state:

For certain applicants who only require a vaccination assessment: You need only submit this page with Page 1 of Form I-693. See Form Instructions - FAQ section for more information.

...so that seems geared toward people in our situation...

You did the proper thing to do. When vaccinations requirements are not complete at the time of the K-1 medical, you have to go see a civil surgeon in the US and have Part 1 + the vaccination page completed and put in a sealed envelope. That is what was done so it's all good.

Send the sealed envelope in her AOS/EAD/AP package.
