OK. you're looking at an "Appeals" form. There's proceedings before it gets to this stage.

this is a form to ask the commission to review a decision made by an arbitrator, or a court decision.

According to the manual, it says i should received payments w/in 14 days of incident.

6.When is TTD paid?
The employer should make the first TTD payment within 14 days after receiving notice of the injury.
Since delays are common, to facilitate the prompt payment of benefits, we encourage the employee to give the employer a written demand for TTD benefits along with the doctor’s note.
If the employer does not pay promptly and cannot justify the delay, the employee may petition the arbitrator to order the employer to pay penalties and/or attorneys’ fees to the employee
Granted, I did not request payments until last Wednesday, which was 16 days post incident (technically 18 since it happened on a saturday but I'm counting from that monday). But now they are just ignoring me.

This is what hubby is supposed to get from them:

c) if the employee cannot work for more than three days because of the injury, the employer must
do one of the following:
(i) Begin payments of TTD; or
(ii) Give the employee a written explanation of the additional information the employer needs before it will begin payments; or
(iii) Give the employee a written explanation of why benefits are being denied.
We are getting no response whatsoever.
It's an employment agency, too, so it's extra confusinnnnng.

1-yes, "delays are common"

2- you'll hear from an insurance investigator

3- the petition you're looking at is not a petition for the arbitrator to look into the case. it's a petition for an appeal (presumably that the arbitrator or court, did not rule in your favor, or to your content)

wait. The employment agency is not responsible for paying you. The employer is.

I'm a sentimental guy. So I called him. He, being a great honest guy, told me that he himself switched to GEICO. I retained him for the other stuff. He said geico won't be able to save me much on the other types of insurance.

omg tyyyyyyyyyyyyy

PS: There is nothing to witness.

Hubby didn't even witness the incident because it just "happened" at some point and he works with frozen meat patties so he didn't even notice until he was out of the cold area and taking a break.

Big goings on here in the local filipina community. A K-1 who came about a month ago, with a son, got married last weekend, and is now booted out by her husband. Wants to return, but has no money.

Husband was 72, and the filipina was 21 with a 6 year old. The woman was talking a lot of trash on FB, and a lot of things don't add up in the story. Don't have any idea how someone in her position can get a ticket home, so I gave my wife the local Catholic Charities phone number.

Should be interesting seeing what happens in this local live drama.

You're quite welcome.

Let us know how you progress.
