Wanting to get a visa for my mother in law from the philippines waht do need to do and who might be responsible for any medical debt?

Well, if your foreign spouse files for his/her mother, that's an I-130.

Since that foriegn spouse is the petitioner, that foreign spouse is also the sponser on the I-864. You could be a joint-sponser, if you want, but ultimately, it's the sponser (that signed the I-864) that's to ensure the beneficiary not become a public charge.

Public charge is a very wide brush - medical debt incurred in the USA is the issue between the hospital and the beneficiary, so if the hospital requires a 'person responsible for invoices' then that can be anyone at the time of medical service.

You can't file anything for your mother in law.

She can file for a tourist visa to visit if she can show strong ties there so she won't overstay

When your spouse is a citizen they can file for their parent. There will be an affidavit of support that will establish responsibility. If there are pre existing conditions medical coverage may be a require for a visa.
