Do I need to file I-693 if I have a copy of DS3025 for K1 visa on AOS

hi fellow vjs. Im on the process of gathering paperworks of our adjustment of status. Ive been doing research regarding whether to complete the I693 or a copy of DS-3025 vaccination documentation worksheet is enough to submit in the AOS package.

Ive seen some post that the DS 3025 is enough but some posts say that I need to fill out part 1 of I-693 and have a civil surgeon transcribed whatever is on my DS 3025. Ive tried calling all the Civil surgeons in our area, but they are asking for $200 which includes physical exam, lab works, vaccinations and paper works. I told them I did all that already in my country of origin .All I need is a civil surgeon to transcribed it in the form. Unfortunately, they refused to transcribed it, they said it is all in the fee. So I am now really confuse if I really need to see a Civil Surgeon just to satisfy the requirement in I-698 Part 1 or take risk and just submit the copy of my DS3025 the vaccination documentation sheet.

Please shed me some light on this before I mail my AOs package.

Did u take all your shots before coming to the states?If nt then u need to take your shots...Well i had 2 shots more to take but all my physical was done again cos the doctor adviced me to...
