medical exam instruction sheet from MTL

hi everyone,

quick question. can someone tell me whether the instruction sheet from MTL was one page or 2 pages. i only have a photocopy of the form they sent me, but i have a feeling that i missed to photocopy the 2nd page. if there's a second page, would someone be kind enough to inform me what i'm missing?

fyi-my interview is on jan 16, 2007

April 27, 07-Filed AOS, EAD and AP

May 14-received NOA with biometrics appt

May 31-biometrics appt for EAD and AOS.

Hey! Yes it does include a second page. I'll post it here mostly about what will happen if you DO test positive for the illnesses they are looking for. This is from the Montreal Package (Recieved in OCT 2006). You did also recieve the list of Dr's correct?

Removal of Conditions:

2009-02-27: Sent I-751

2009-03-07: NOA I-751

2009-03-31: Biometrics Appt. Hartford

2009-07-21: Touched (first time since biometrics) Perhaps address change?

2009-07-28: Approved at VSC

2009-08-25: Received card in the mail

Naturalization

2012-08-20: Submitted N-400

2013-01-18: Became Citizen
