What do u mean, i thought this form was to be filled with the beneficiary information not the petitioner

Timeline

Passport Book Number = Passport Number of the applicant, ( If I remember correctly DS-160 has 2 fields passport number and passport book number) some countries have passport book number, if applicant's country does not have passport book number than leave it blank.

All amounts should be USD.
