Passport name change (maiden to married)

To change an Indian passport name, you need

1. attested copy of marriage certificate

2. copy of husband passport OR affidavit from wife/husband

I am the USC husband and am back in the states... I have the original marriage certificate with me (to send to NVC) but wife does have copies (though not attested). wife already has copy of my passport and I will be in the US while she goes through this process

some questions about this process:

has anyone gone through the name change process in india? if so, what was your experience?

where do you get documents attested?

what happens to your visa stamps in the previous passport?

do you still list the spouse name in the passport?

To Change name

You need to notarize it. Speak to a lawyer, Also the old name and new name need to be published in Newspaper. 1 in English and 1 in Local Language. If you want to change the name in the passport then you have to go thorugh the passport process. But that is not required. It can be done when the passport is due for renewal.

Maiden to Married last name:

Process is pretty simple. Just apply for re-issue passport in any nearest PSK ( Passport Seva Kendra ), and make sure to bring original marriage certificate. And they will re-issue the passport with married name.
