Inexpensive Civil Surgeon Needed in Kentucky

Hi everyone. For those in Kentucky, please i need help getting a civil surgeon for my form I-693 for AOS. Already have my vaccine records from K-1 visa, just need it to be transcribed by a civil surgeon but am having trouble getting one in Kentucky. Called a few of those listed on the USCIS website but some want me to undergo full testing again while others say they only transcribe for Refugees. Any suggestions about civil surgeons in other states that can do this via fax is also welcomed. Thanks

It cannot be done by a fax. You must see a civil surgeon in person.

Hi! Not sure how far you are from ohio, i got one by the name of Dr. davin 513-241-4135. he only charged $75 to complete the 1-693 form for us as long as you've completed your vaccinations. however, they also have vaccines in their office in downtown. Good luck.
