1. Documents to bring during medical exam at Saint Lukes Manila.

2. Documents to bring during interview ( documents from my Fiance).

3. Documents and requirements to bring during CFO seminar.

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

7. Form 1040

8. Employment letter

9. Payslips

10. Bank account statement

11. 2x2 visa photos (3 pieces)

12. Original NSO birth certificate

13. Original NSO CENOMAR

14. Original NBI clearance (Travel to US)

15. Original and photocopy of Appointment letter

16. Original and photocopy of USEM letter

17. Original Letter of Intent to Marry

Haven't don't CFO seminar yet so I won't be able to give you information regarding its requirements. But you may visit their website for the documents needed. Good luck on your journey! :)

St. Lukes Medical Exam
  • Valid passport
  • Photocopy of Passport Biographic/Data Page, the page in the passport that contains the applicant's photo and information. The photocopy should be the same size as the original, not enlarged or reduced. (vertical position, short bond paper)
  • 3 pcs recent 2x2 visa photos (2-USEM, 1-St. Lukes)
  • Appointment letter from the embassy (2 photocopies)
  • NVC Letter (2 photocopies), if available
  • Online Registration form (2 copies)
  • Fee: $223 (Paid in Peso at the prevailing exchange rate)
  • For divorcee/annulled,do not forget to bring your annulment/divorce papers
Commission on Filipinos Overseas
Attendance to Guidance and Counseling Program
  • Fee: P400 for the Counseling & Registration
  • 2 valid ID's with photograph and photocopy of each
  • Duly completed guidance and counseling form (provided at the office)
  • Other documents as may be required by the counselors*
Registration (if VISA is "in hand" already)
  • Original and photocopy of passport
  • Original and photocopy of visa
  • Original and photocopy of CFO guidance and counseling certificate
  • Duly completed emigrant registration form
Additional Documents (whichever applies to you and if you want to bring any of these):
  • Notarized parental consent
  • Notarized parental advice
  • Bring parent's ID and a photocopy
  • NSO Birth certificate
  • CENOMAR
  • NBI clearance (can also be used as an ID)
Petitioner's Documents (Photocopies):
  • Passport bio page
  • Birth certificate
  • Final divorce decree(s), if applicable
  • Proof of singleness, if applicable
  • ITRs and other financial documents
  • Bring recent proofs of bonafide / ongoing relationship like pictures, chat logs, remittances, Skype call logs & print screens,etc..

KNOW YOUR:

  • Fiance's phone number, complete address including zipcode,etc..
  • Fiance's full mother's maiden name
  • Close Friends and relatives phone numbers, email add (fiance & beneficiary) can be used if fiance or beneficiary's family don't have a landline or e-mail
  • Fiance's neighbor or landlord/lady's phone number (if fiance don't have a landline)
  • A relative's address and phone number in the USA if any
  • Childrens name, age, DOB, grade level, pictures, if applicable
Familiarize the next immigration procedures
Other requirements maybe required for those who were previously annulled or divorced and for CR1/IR1
Bring the other documents you have or that might apply to your case/situation
Manila U.S. Embassy Interview
Part 1 - At the Tent
  • Appointment letter from USEM (interview confirmation letter)
  • Valid Passport
  • DS - 156 (2 copies)
  • DS - 157 (2 copies)
  • DS - 156K
  • DS - 230 (fill-out PART 1 only) & Supplement Page if any (you might not be asked for this but just be ready)
  • Bring extra 2x2 visa photos (for DS-156)
Note:
*You may attach the 2x2 pictures on DS 156 and sign (DS 156) to avoid delays
* You may need not to provide another 2x2 pics since we already provided 2x2 pics during our medical exam.
(Done with the medical exam at least 1 wk before the interview)
You may bring your chest x-ray CD, Vaccination worksheet & medical exam receipt from St. Lukes*
Note: The greeters will ask you if you are done with the medical exam already. Also check for St. Lukes stamp on your passport.
Part 2A - Filipino Consul

ORIGINAL DOCUMENTS

  • Passport in a plastic ziplock*
  • Birth Certificate
  • Certificate of No Marriage (CENOMAR)
  • NBI Clearance (abroad purposes / USA Visa)
  • Police Clearance, if applicable (if you had lived/worked abroad for more than 6 months)
  • Pay Visa fee $240 (bring the receipt)
Part 2B - Filipino Consul (Financial)
  • I-134 Affidavit of Support form (with original signature)
  • Photocopy of most recent Income Tax Return (form 1040)
  • Photocopy of most recent Wage statement (W2)
  • Employment letters stating position, salaries, tenurity,bank statements, recent pay stubs, etc (you might not be asked for this if the income is above the poverty level)
Part 3 - American Consul
Might ask for the following:
  • Letter of intent to marry (Petitioner & beneficiary with orig signatures, addressed to Manila USEM)
  • Final divorce decree(s)
  • Recent proof of ongoing / bonafide relationship: e-mail, chat logs, call logs, remittances, pictures, etc
  • And other docs that might apply in your case/situation
These might be asked by the Filipino consul too
Additional Documents you might want to bring:
  • Petitioner's passport and birth certificate photocopies
  • Replica of I-129F packet
  • Original copies of photocopied docs send in I-129F
  • Bring NOA 1 & 2
  • USEM Letter if available*
  • NVC Letter
  • Bring BLANK DS forms (incase u make a mistake and needs to fill out the DS forms again)
Make copies of ALL your documents

1) SLEC has everything listed on their website, link in my signature below

2) The embassy instructions letter has everything listed that the embassy requires, including info and a map for St. Luke's (in link below)

3) CFO website is sort of vague as they use the "other documents" clause all the time at CFO, a list of the most common "other documents" is in the link below.
