Which name to use in forms?

I friend of mine, who got a permanent residence last year, advised me to filled the forms with my husband's last name, because it will be easier to change my name in my documents eventually ( I will get rid of my last name to get his). The problem is that I had my medical exams and the I-693 Form filled out 10 days before I got married and I filled with my single name. My question is, is that a problem if I send the I-693 with a last name different than the other forms? Should I go talk to my doctor and ask him to update this information?

It's not a problem. File using your married name. You don't need the I-693 changed.
