Hi

I am under K1 status and I am gathering everything for AOS, .

I went to a civil surgeon for my vaccination supplement form I-693. I didn't have DS-3025 but the surgeon accepted to fill in I-693 as long as I provided my French vaccination record with a translation, which I did.

The surgeon filled the form while I was in the waiting room, so I have no idea what he wrote, and he didn't bother to let me know. Then the secretary handed me an envelope without a word about me being fine or not. I assume it means I am all good, so I didn't ask whether my vaccinations were fine or not. I asked if I should keep it sealed and she said yes.

My question is: should I really keep the envelope sealed or not? I kind of want to see what that doctor wrote!

Does form I-693 always come in a sealed envelope?

Hi,

I suggest you do not open the envelope!

If you are extremely curious, go back to the doc's and ask them.

Other then that, I am sure you're fine. If there was an issue with your vacs they would have told you, if not for the gov. but for the doc to make more money off of you!

I would say you are required to submit the "form". So, if the envelope is sealed, how else will you know if whether or not the "form" is inside?

Seriously, there's no mystery here as far as I'm concerned. You are the patient of the civil surgeon and you are entitled to know what he wrote about you. Therefore, it is up to you to determine if he has been erroneous, or not. I think if the USCIS was that concerned they would clearly stipulate that you provide a 'sealed' envelope from a licensed surgeon, health professional, etc.

sealed envelope means sealed cannot be open, I-693 sealed envelop is like the petitioned documents we received along with our visa's, enter US and surrendered to an Immigration officer at the POE remained sealed.

If your curious about what the civil surgeonput into your sealed I-693 form, u could asked for a recopy of what they gave to you. i don't know if they asked for payment to that, but i had no diffculties asking a copy for me for free.

Hi, I'd made sure and sent it sealed, I don't have time right now to find the exact description right now, but I have seen the vaccionation supplement described as needed to be sealed by the civil surgeon. Like others have suggested, go back and ask for a copy, although I'm not sure they kept that for their records if you're not a regular patient. Either way, it's best to keep it sealed in my opinion.

Um....yes...you KEEP it sealed. If you want to know what the dr wrote, call them and ask for a copy of what he/she wrote. The CS we used gave us a copy of what was sealed in that envelope.

Ok thanks. I guess I won't open it then. After all, I am pretty sure that I am fine. I had all my vaccinations updated when I had the medical back in France. I imagine the surgeon would have told me if I had to take an extra shot.

I did have to pay 50 dollars there. I don't think they kept a record of this but I can't be sure. I have all my papers ready, so I will go ahead and send the AOS application on Monday without asking the doctor first.

Mine was sealed and it was stamped across the flap. It also said do not open. LOL,

I'd say don't open it. The office also gave me a copy of what was in the file. So if you really need to know, give them a call!

They are supposed to (as per CDC instructions), give you a copy of the vaccine record for your personal use. I'm sure if you ask they will give you a copy. I know the doctor that I went to, made up a medical file for me, so it's quite possible that they will have a copy of it at the office.

The I-693 is very similar to the form DS-3025. The doctor told us that it's sealed because it's so easy for anyone to fill out themselves, that having it sealed ensures it's from the doctor (civil surgeon). I wanted a copy for our records (just in case it was lost) which they gave us, but she said that we would need another sealed form from them if it were lost.
