Vaccination Supplement: argument with civil surgeon

The DS-3025 is "Instructions to Panel Physicians for Completing Vaccination Documentation Worksheet". K1 applicants who have not had vaccinations at the consulate medical exam would not have received this form. So how could it be necessary to show that in order to get vaccinations in the US? Doesn't make sense to me . . .

basically you GOTTA get a copy at your physical. And bring your GP's vaccination record fo your shots!

IF you had the vaccinations done at the medical . . .

And therein lies the problem. The medical clinic in Ciudad Juarez doesn't give the applicant ANYTHING.

I suppose it's possible but seems doubtful - they are not exactly helpful or nice people. But I found a local CS who said it would be no problem to give the vaccinations (no medical) although the price was high. I'll have to find out how he would handle transferring a Mexican vaccination record onto the correct form.

1. David presented proof of his vaccination record to the panel physician in Frankfurt. He actually had to have his blood drawn by his GP to prove his titers as his vaccination record in the UK was long gone...

2. The panel phys determined that he needed a tetanus booster before he could be signed off on his vaccinations

3. David returned to his GP, had the booster, and the GP faxed the record of immunization for the booster to the panel phys.

4. The panel phys updated the DS-3025 worksheet and faxed the GP a copy of it. David picked the form up from his GP.

Since David's medical exam is current, all he needs is an I-693A completed by a civil surgeon. Since his vaccinations are up to date, the civil surgeon needs the DS-3025 (proof) to complete the I-693A.

Does that make sense?

All makes sense. But the one big difference is that David was doing his vaccinations while in Frankfurt and dealing with the panel physician there. If my fiance does his vaccinations in Mexico (much cheaper) than the CS will need to transfer that info to the I-693A without having the DS-3025 worksheet since we won't be dealing with a panel physician. It's ok - we'll wait and see what happens . . .

Right... David's vaccinations were covered by his health plan, so we didn't have to pay anything to get him 'up to date'...

I received the DS-3025 worksheet at the medical and enclosed the copy with the AOS. I didn't send the supplement in with the application figuring I would bring it with me to the interview. I took both the DS-3025 worksheet plus my documentation about my immunizations received over the years to the Civil Surgeon to transfer the information to the supplement. He had difficulty reading the Panel Physician's handwriting on the worksheet - so he didn't even use it; he transferred the information from my original documentation instead. As long as the information is in English or the CS can understand Spanish, the documentation from his own physician should be acceptable. (And after all that, I was approved without an interview so still have the supplement!)
