Timeline

During the adjustment of status process, it is my understanding that my fiancee won't have to do another medical (if medicals is less than a year old) but just have some sections os form I-693 filled. What will she need to show a civil surgeon before form I-693 will be filled? Form DS-3025?, vaccination records? Was she suppose to be given a copy of form DS-3025 (or vaccination records signed by the doctor) while in Ghana by the doctor who did her medicals? Does Akai House (where she did her medicals) give you a copy of medical records? I want to make sure she brings along all necessary documents before she leaves for the U.S. Thanks.

You should have received a copy of the DS-3025 vaccinations sheet. Please see this post to know if you need to see a civil surgeon or not. They told my fiance that they didn't know what that form was, so I had him print it and show it to them. Then they told him that they don't give people that form and that the embassy would give it to him. I'm wondering if it's in the packet they give them when they go to pick up their visa, but I honestly don't know.

Thanks Em&Stephanie. I think it is in the packet but I don't think they will given her a copy at immigration here in the U.S. I forgot to tell her to ask for a copy at the interview. Now I don't know how she can get a copy if the doctor won't give her a copy.

If they do not give you a copy, then one would be in the sealed envelope you handed over at your POE. In order to skip the civil surgeon you would have to be absolutely certain you had all the required vaccinations at the time of your medical and that they were correctly marked off on the form. You could then state in your cover letter that you had all the required vaccinations and a copy of your DS-3025 is in your file with your other medical records. If you do not know if you had all the required vaccinations, then you will need to find a civil surgeon that will not insist on a new medical and get any missing vaccinations and have the rest transcribed onto the I-693. Take any vaccinations records you have with you to the civil surgeon.

Em was so nervous and excited at the interview, he just forgot to ask about it. The hospital did give him a copy of his vaccination records (just not the form DS-3025), so hopefully that will be helpful. wacko.png
