Past drug use

I honestly wrote down that I used to smoke 11 years ago. I quit when I was pregnant. I wrote down all my hospitalizations and surgeries including the one on my nose when my horse kicked me in the face when I was 13.

While this is from medisys in Montreal I suspect that the other forms are similar for other panel physicians.

I'm not sure if anyone else has taken pictures of the forms before?

Are those forms from Vancouver or Montreal?

The embassies don't ask you medical questions - the Dr at your medical does . These look like they're from Medsys, one of the panel physicians.

My 2 cents - I smoked pot when I was 19/20 - 17 yrs later (6 yrs ago) when I was immigrating to the US I cannot remember if I was asked, but I likely said no. Did I lie, I suppose so but at that point it was water under the bridge, I hadn't done it in 17 yrs and frankly there's no proof I did anyway (unless my college room mate pops up!)

I know, firsthand, that Dr Cheema (Vancouver panel physician) is a stickler. A note from her doctor (as suggested by Awkwardsoul) would be a VERY good thing for your fiancee to have in her back pocket, IMO.

Its a good practice to have a letter from your family doctor explaining any conditions he/she is treating you for, any current medications you take etc... so you can give it to the panel physician. I had letters from our family doctor which included that info along with how long he'd been our doctor and my overall health in the last 5 yrs.

I was asked at our USCIS interview if I had ever used illegal drugs of any kind at any time in my life, not at medical. Not that I had to worry about what I was going to answer, but I had already been warned by attorney that there is only one acceptable answer. It is a black and white, right and wrong response they are looking for...

I'm not worried about 1. because she is going to Vancouver.

And 2. she has no criminal background of drug use, but was admitted to the hospital because something went wrong one time and caused a bad reaction. Basically why she never used it again. Will the hospital be an issue? I don't know what to do.

Why try and figure out if there was even record of what caused her "bad reaction" or whether USCIS will access it. Could have been food poisoning for she knows or remembers.
