I read somewhere these should be blank, her passport sasy "DFA MANILA"

do I put that in both fields?

I am prefilling, I messed up a date when i originaly sent it over so I am fixing it now and 2nd guessing everything else while I am at it lol.

Also for her city of birth which is Columbio Sultan Kudarat. It only fits "Columbio Sultan Kudar" so do I aabreviate and put "Columbio Sultan K." ?

State/province - Yes put Manila if that is what it states on the passport.

For city of birth, I guess that will have to do. Not easy to use an attachment with this form.
