Hello everyone!!! I am K! visa holder, getting ready to send my AOS package this week.

Could anyone please tell me if the birth certificate has to be translated by a certified translator or can anyone do it? Does it have to be notarized?

Also, about the medical examen....do I need to send a copy of the vaccination sheet that my surgeon filled before the interview at the embassy? Or they are supposed to have he original ones?

All documents have to be in English or translated; as for the medical "thats kinda a universal document, I didnt".

But the birth certificate needs to be translated and by an ATA, INS certified translator. I used FoxTranslate, now Rev.com; scan document, pay online and receive certified PDF.

I included my vaccination sheet (and only that), as this is specifically listed in the I-485 guidelines.

06-13-2012 - I-129F sent

10-11-2012 - NOA2

01-09-2013 - Interview (approved)

01-18-2013 - Visa received by DHL

Yes, it needs to be translated, but no, it does not need to be done by a professional 3rd party. Anyone fluent in both languages can translate. Whoever does the translation just needs to include this certification on the translated document > http://www.visajourney.com/content/translations

If you had all the shots at the time of your medical and they are marked off on a properly filled out DS-3025, then just send a copy of the DS-3025. I just have a paper that says "vaccination documentation worksheet" , for use with DS2053...

I don't have the DS2053 itself, but from what I understand CBP at the POE send it to USCIS and they should have it right?

Last one: the box "vaccine story complete"...does that mean that is complete for the K1 application, or it's all set for AOS? (I got all the shots in Argentina just in case)

Correct. CPB takes your envelope with all the info, including the medical and forwards it to USCIS; so when you send your AOS application, they should already have it. Are you using the sample forms to complete your application?
