Hi all - just for clarification, if my medical has already been completed in London prior to my leaving (had medical just a couple of months ago) and I have since had the required vaccinations which were missing at the time, I need only submit pages 1 and 5 of the I-693 (signed by the civil surgeon, of course), right? USCIS has all the other information in my file.

Think this is the case but could someone else just OK what I'm saying, just to be sure?

Yes that is correct, I had to find a Civil Surgeon to complete the vaccination section and they charge me $50 each for myself and 2 x K2 children.
