The letter of intent does not need to be notarized. It also does not need any addresses on it. 'To Whom It May Concern' will do. It needs the main, very important statement of

I, (applicant’s or beneficiary’s name) , do hereby state that I am legally able and willing to marry (petitioner’s or beneficiary’s name), and intend to do so within 90 days of my arrival into the US using the K-1 visa.

Then you sign and date it.

There is no debate about original signatures or scans/copies. An original signature is required. If you send a copy instead, then you are taking the risk of getting an RFE. Does everyone get an RFE for not having the original signature? No, but many do. Why risk it when you can simply provide the letter with an original signature on it?
