Hi Good day.

I just have a quick question here, do I need to have another shots and meet a surgeon here in the US for filing an AOS even if I already had my shots before coming here in the US? What documents do I need to prepare for filing AOS?

You do not need to undergo another medical examination for AOS provided your medical exam (from K-1) is within 1 year of filing your AOS. You will however, need to see a certified civil surgeon (see USCIS website for a list) to do a transcription of your K-1 medical exam vaccinations and other records for the I-693 form. This form has to be signed by an authorized civil surgeon who also has to make an initial determination, based on your medical exam records, on whether you are admissible or not on the basis of public health grounds. This form is keyed in by the civil surgeon and placed in a sealed envelope to be attached to your I-485 application. The transcription fees can range from $10 to $50 depending on where you live. I suggest you shop around before you make a deal. There are tons of unscrupulous docs out there that insist on your getting the USCIS medical exam again even if you don't need it.

Thanks for helping me out. I am just so confused if I have to see a civil surgeon or not because I already got shots when I was still in the Philippines.

As it states in the post I linked to, if you had all the required vaccinations and they are marked off on a properly filled out DS-3025, as described in that post, then you can just send a copy of your DS-3025.
