I checked off BIRTH IN THE U.S

What do I put for the rest of both boxes?

(Give number of certificate, date and place it was issue)

( Have you obtained a certificate of citizenship in your name?)


SO CONFUSED!!!

You dont put anything after that because you were born in the USA the other part is if you were naturalized citizenship.


K1 approved 06/13/2013

AOS approved 02/12/2014
ROC approved 09/14/2016

DIVORCED 01/10/2020
