Immunization worksheet

My fiance received the immunization sheet prior to his medical. When he did the medical, he received some of the immunizations that he will need for when he is in the States. When he went to the interview they took the medical sheets as well as the immunization worksheet. My fiance has his visa now but did not receive back the immunization worksheet. Is it in the sealed brown envelope? Was the embassy supposed to give it back to him? How does he get it to show to the civil surgeon when he comes to the states?

The form he's looking for is called DS-3025.

Some consulate physicians give out a copy and some don't. Weird.

About the best he can do now is phone them and see if he can get a follow up copy.
