Hi guys,

I'm adjusting my status from a fiance to a spouse of an american citizen. Since my full medical exam was done in November of 2012, I went to a civil surgeon today to transfer records from my DS-3025 to vaccination part of form I-693 ( page 5). While setting up an appointment with the civil surgeon on the phone, I double asked if they would be ok with just part 5 of I-693. They seemed to be cooperative and said that it was not a problem and they would not need me pass another full exam.

Today, when I came over to the doctor's office, I was checked-in, filled in all forms, they took my vaccinations info + Instructions for form I-693.... and told me that I would have to pay 390,00 for immigration check-up, then titers for this and titers for that. And blood test, and what not. And that the doctor CAN NOT fill in only part 1,2 and 5 of the form, because he would lose his licence. When I showed them instructions and provided evidence, they said that that was how he always did it and it was the only option possible. They even wanted to do me an X-rey again.... Well, I cancelled my appointment with them, went to local Health Department, got my MMR-2 and Tdap for FREE, shared with them the story of my visit to a civil surgeon, and they agreed he tried to rip me off.

Now, my question for you all is: If I have my copy of DS-3025 ( another one was in the brown exam packet that was turned in by me at my port of entry, that is New York), it does not have anything marked off at the results section. Health Department person checked my immnization and said that to be on a safe side I could do MMR-2 and Tdap, which I did. Will it be ok to attach copies of immunization papers from Health Department to DS-3025 and just send it with my AOS package? Or I still have to go to another civil surgeon and ask them to transfer my data to I-693? Thanks

I take it your vaccinations are not marked "complete" on the DS 3025. If they were, then you would need nothing else.

Otherwise you need the I-693 vaccination supplement. You can call around to other civil surgeons and see what they charge. The doctor CAN fill out only partial parts of the form provided you had an overseas medical exam in the last 12 months. The doctor is stating his policy (to make extra money) Many do this.

You can also send in the I-485 without the I-693 and get the I-693 in the meantime. You will get an RFE and you can send in the I-693 at that time.

Incidentally, Alla and I worked extensively with Senator Patrick Leahy in 2010 to get legislation drafted to allow you to go to any US physician to have this done. Civil Surgeons are worse parasites than attorneys in this process. It bounced around and was attached to various legislation that did not get passed. While I am not in favor of the immigration bill just passed by the Senate this is included in that bill. We will see if it survives the legislative process.

If a doctor can give you a prescription for opiates, he out to be able to sign off on a measles vaccine. Removing this from the monopoly of the Civil Surgeons would save everyone a lot of trouble and money. Senator Leahy strongly supported this and his office drafted the legislation. He has an outstanding immigration liason, Susan Sussman, who met with the Senator and Alla and I and she did a great job working on this.

Thanks a lot for your reply! Yes, my DS-3025 did not have "Complete" marked. I called about 8 different offices and finally found a civil surgeon who agreed to transfer my vaccination and to transcribe new ones I received at Health Department for 75$, I was very glad about that. She marked my Vaccination History as "Complete" now and I sent out my I-485 yesterday. I agree with you that many civil surgeons just try to rip you off.
