Hello guys...here is my problem!i had my medical exam a month ago (for K1 visa) BUT i cannot find th DS-3025 copy and I need it for AOS..so do you know if the hospital is kind enough to give me a copy?otherwise any suggestions?Thank you so much!

If you are still in Greece, why dont you try to contact your DR? That who performed your medical examination? The thing that the original one is inside the seal envelope. MAYBE, if you talk to him he can help you with it. Otherwise, you need to go to a civil surgeon once you are here, and ask for a transcription, but you will need to show your vaccination record and pay some extra money

hi!thanks for your reply..i will call the hospital 2morrow..I'm in th US but I did however bring with me my vaccination record booklet from Greece, where it states all the vaccines I have taken throughout my life, including the ones I had to take for the K1 visa.

Your welcome! It's like $20 if the surgeon doesn't to performe anything else. I know this because, a lot of people here need to do it. Unfortunately, a lot of people here had being victims of immigration officer/ uscis, by getting the package we gave at the POE, lost. The package has everything about our process including the medical results. What happens next? You get an rfe asking for your medical results, which you had already, but even when you didnt lose it, you have to pay $200 to get a new medical result. Thank God it didnt happened to me, but at least you know what to do in case something happens. Its clearly not your case, but my point is that civil surgeon here are used to deal with lost/ stolen immigration (medical) documents. Go to the USCIS webpage and you can find a qualify civil surgeon next to your home

so I'm asking only for a transcription for AOS package right?Once the Civil Surgeon has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification) or do i need to include all parts of i-693 form?hanks again!!

Usually the Dr fiil out that for you but ask him first!!!

Timeline

If I'm not mistaken, the DS-3025 was in the brown envelope you surrendered at your POE and is now part of your file. However, just be safe, we e-mailed the hospital that did the exam and they e-mailed us signed and certified copies of the DS forms given to the embassy which we included in our AOS and that included the DS-3025.
