I-693A (vaccination supplement) - Where to Find It

My wife and step-daughter will be coming to the U.S. on a K-3/K-4. I'll file for AOS as soon as they arrive. Need to include the I-693A (as opposed to the I-693), but could not find it on the USCIS web site (on the forms page, I only see the I-693). Any ideas on where I might find/get the form?

The I-693A is provided by the US Civil Surgeon. If you try google you might find a blank pdf somewhere but it would still need to be filled out by the doctor.

did your wife get a copy of the completed medicals from the examiner in columbia...? if she didnt see if she can contact the examiner's office to see if she can get a copy of the examination results that she took to the embassey for her interview...

all they need is a copy of the vaccination sheet..when i sent my AOS in i stated on the cover letter that the original was handed in at POE..but i sent a copy of it in the package...

We received a copy of Darcy's medical when he had it completed in September. Does this mean I can just include that instead of having to go to a doctor here and get another medical done?

I think so, unless it was incomplete and you needed more shots, then you'd need an updated sheet signed by the civil surgeon.

I could be wrong on this one though.

Some have gotten away with submitting the medical forms they received in their country and some have received RFEs for the I-693A. There hasn't seem to be any rhyme or reason for those that were RFEd and those that weren't. We only submitted the DS-3025 (??) with the AOS documents and were fine (and took the I-693As with us to the AOS interviews...even though they weren't asked for!) yet I know some have posted they had to end up sending the I-693A with the AOS. David didn't require any shots, but Chris did and we submitted the same documents for both.

The Civil Surgeon should have copies of the I-693A and any other necessary forms so you shouldn't need a copy. Just find the list of them on the USCIS website for those in your area.

Thanks to all for the good advice. I'm going to try to have my wife get a copy of the medical exam...assuming it can be done (not sure yet if the Bogota embassy requires the exam to be delivered in a sealed envelope, in which case copying the report would not be possible). One thing is for sure...the Form I-693A is not on the USCIS web. The link in the previous post brings you to another link to the I-693 only....which is what I also found when I first did my search.

Thanks very much. I was hoping to be able to have my wife take the form with her to the appointment with the designated physician in Bogota...so that he or she could complete it based on her medical records (which, of course, are in Spanish). But it seems as if the form can only be filled out by a U.S. doctor who is a "civil surgeon." I guess I need to find a "civil surgeon" who can read Spanish...or get the immunization records translated into English.

I'm thinking about trying to get a copy of the medical exam from the Bogota doctor to submit with the AOS form....but I might end up getting a request for the I-693A anyway. Hmmmmm.......
