Hello everyone,

I arrived in the US on a K1 Visa June 24,2013.

I lost my I-797 Notice,do I really need a copy of this notice to apply for my green card? I took medical examination in China already but I did not get vaccinated. I went to a civil surgeon to complete my shots and they said they charge a fee of $165 to fill out I-693 in addition to the cost of the vaccinations. If I only need vaccinations, which sections of I-693 need to be filled and submitted along with my application? Do I need a surgeons signature and stamp on I-693 if I do not need an examination? Please advise.

You could try using a printout of the email from USCIS that stated your I-129F was approved, or the letter from the NVC that stated it was approved and being sent to the consulate instead of paying the fee. Just note on your cover letter that you do not have a hard copy of the I-797 approval notice and are using those instead. Some others have success with that in the past.

Yes, the civil surgeon must fill out page one and the vaccinations page of the I-693 and sign off on it. They will place it in a sealed envelope for you to send with your AOS paperwork.

You arrived on a K-1 visa, not a tourist visa. So status is "K-1" or "K-1 visa holder".

K-1 Visa Timeline AOS Timeline

- Aug 31st, 2011 - Mailed I-129F package - May 29th, 2012 - Mailed AOS/EAD/AP package

- Apr 13th, 2012 - Visa received - Aug 24th, 2012 - Green Card received

ROC Timeline

- May 19th, 2014 - Mailed ROC package to CSC

- Aug 8th, 2014 - Green Card received

N-400 Timeline

- Dec 29th, 2021 - Filed online. Got notice that biometrics will be reused.

- Now waiting...
