MMR vaccine and DS-3025

Hi all,

I am filing AOS papers now, as I got married 3 weeks ago. As K1 visa applicant, I got my full medical in Ukraine in November, less than a year ago, and they gave me DS-3025 vaccination form. MMR vaccine has only Rubella marked, Slots for Mums Mumps-Rubella, and MMR, and Measles Rubella are empty. In the results section, panel physician wrote visa K, without marking anything else. There is no check mark in the line that reads vaccine history incomplete, as well as no check mark in the line that reads vaccine history complete. What should I do? Just send me my DS with AOS papers, or go to a civil surgeon to get missing parts of MMR ( measles and mumps).

You need to find a civil surgeon to get your missing vaccinations and have page 1 and the vaccinations page filled out on the i-693. You do not need another medical, but you will probably have to call around to find a civil surgeon that will not insist on giving you one. try looking through this thread for any in your area> http://www.visajourney.com/forums/topic/231026-civil-surgeons-who-dont-rip-you-off/

thanks a lot for your reply. I've been to a CS today, and he tried to charge me 390 for immigration exam + titers +blood test and so on and so forth. I had called him before the visit took place, asked if he would do only Vaccination part. His assistant said "ok." I guess that was their way to make me come over there. I cancelled my appointment, went to Health Department and got my MMR-2 and Tdap for free. Do you think it is ok to attach receipts about those two vaccination to DS-3025 and just send them with other AOS papers? ( skipping Civil Surgeon part).

No. You will need to call around to other civil surgeons to find one that will transcribe your vaccinations onto the I-693.
