Back in my day, these were store bought monsters with double chambers and hoses connecting the two chambers. Ice in the bottom with Scope mouthwash in the top gave a cool, refreshing hit. In addition to needing help lighting one, the first toke would burn the whole bowl and yet none of the smoke made it too your mouth. You had to exhale and take a second toke to get the smoke.
