DS-3025 need dose #2 withing 4-8 weeks of medical, will it give us RFE if not done?

So DS-3025 Vaccination Documentation Worksheet given to my husband by doctor says as results that the Vaccine History is Incomplete and that Applicant may be eligible for blanket waver because vaccinations not medically approprite so the check marks on there is for Insufficient time for Varicellaand Influenza. Both were given to him at his medical in May this year.

Appendix C. Vaccines to Complete for adults, for vaccine Varicella dose #2 it says it needs to be given 4-8 weeks after dose #1. Dose # 1 was given to him on 5/21/2013. So the queston is what should we do now. Should we send a copy of that paperwork with AOS and is there another form we have to fill out to send that with. Will we get an RFE if we do not have the #2 vaccine done within the 4-8 weeks specified time for timeframe for dose #2?

Only one dose in a series is required for AOS. If the DS-3025 is properly filled out and at least one in a series of vaccinations was marked off, then it should be ok.
