My Middle Name - very confused!

Timeline

Okay so I was born with no middle name.

I always liked the name Isobella, so for my birthday my Mum legally changed my middle name to Isobella by deed poll.
It is on my driving license but not on my passport.

So my question is - should I include it on the forms? We are in the last stage (at the US embassy in London) and didn't write it on the original application because it is not on my passport.

I think my police certificate will say my middle name on it because I used my driving license as proof of address and I am worried that it is really going to confuse the situation. On the forms for the US embassy (packet 3) I have written N/A in middle name and I worried that that is technically lying and obviously I don't want to do that - those forms haven't been sent yet so could change them.

Technically I have a middle name but I do not use it on anything official except my driving license.

Any advice?

That is a good idea actually, but still doesn't solve the problem that the original forms do not have the middle name on....

I honestly don't see this becoming a big problem -- I am sure that the consular officers are willing to let you explain why you have a middle name on some things and not on others. So long as your first and last names are correct, as well as your birth date and other info, I really doubt anything negative would become of the situation.

My advice, in this case, would be to not put N/A, but to put (See attached explanation) and to explain the entire situation on that.

I have the deed poll and I was planning to take it to the interview with me to show the embassy person. And maybe explain there? Or do you think I should include with packet 3 forms, the explanation?

Okay so no forms are to London. Then I would definitely put Isobella on the form under other names used, ie Mary Isobella Smith on the other names line.

Explain it at the interview to the consulate officer. That's not the first window you are called to, which is a document collector/organizer. The second time called is the officer who does the interview and approving. No guarantees anybody reads or cares about visa matters explained in letters. They are just filing papers and scheduling interviews. Might as well save it for "in person" with the one who actually looks at your stuff and evaluates it all.
