What's "port of entry " on the address change form

Hi:

I 'm about to change my address via mail and I currently hold 10 year GC card.

On the form ,it asks for "Port of entry".

Is this the time when I entered the USA with my CR-1 visa for the first time( Apr 2010) or the time when I went to Europe for vacation and came back to USA and entered the country from LA INtl Airport again (Nov 2011)?

Which one would be the Port of entry?

if you read above that section it says "Last entry into the US" so you would put the place where you last entered the US..
