I-134 for 2013 - only letters and space

I started filling another affidavit of support since there is this new version since 12/17/2012. And in the for example Middle name i should put N/A and it is not allowed, for State also, for annual income only numbers (example 55700). Should i put only numbers like that 55700 or what, and leave middle name and state blank?

Ok this is the Nth time I've seen this example form linked. It is just an unfilled form for me. Is this so for everyone else? If it is then it is a horrible example form.

And with regards to your question, I typed in adobe what I could and hand printed everything else if some thing wouldn't fit in a space.

i have this form filled out, but the form on VJ is older version, i downloaded new version, and for example Annual income you can't put 56,700 but only numbers 56700. And I can't out N/A. That is what is confusing me.

For middle name, characters are not accepted, just the alphabet... maybe type none (that makes me nervous, some idiot could give you a middle name - "none") or try leaving it blank.

Income, again no characters, just numbers

It is not blank for everyone. It is a filled out form used as an example of how the questions should be answered. Perhaps you need to update your adobe reader or check the pdf reader plugins for your browser.

The form instructions do not state not to leave anything blank, so you can probably just leave them blank with no problems. As for the income, just enter the numbers without characters. No comma needed.
