Hi guys,

I am trying to fill in I-693 form and I have a few questions:

1) What are civil surgeons fees?

2) What last name should I provide in the form? My maiden name or my married last name? I had my medical done in Ukraine in November, and, of course, it had my maiden last name on it. So, what last name should I go with now?

3) I have 2 vaccines marked as absent Measles -Rubella, and Mumps-Rubella, even though I told doctor back home I had them. What should I do now to prove that I had had them before? May I just sent this I-693 form without going to a civil surgeon and askign them giving me a signed and sealed envelope?

1) A civil surgeon is a private doctor who can charge any fee he wants so call around. Some may say you need a whole new medical. That is incorrect, so call another one. Your Ukraine medical is good for 12 months. At most you only need the immunization part filled in if you are lacking something.

2) if you are using a new married name and are applying for a greencard in that married name, then use that name on everything, even the I-693. Adjustment of Status is like starting a new process so use the name you plan to be called in the US.

3) mumps, measles, rubella could have been covered by one shot called MMR. Do you have that marked on your sheet?

Thanks for your reply! So, when I will be sending to Chicago my whole AOS packet, should it include only page and pagr 5 of medical exam form? Or it will also have empty page 2, 3, and 4?

If I use my new name, will it be ok that it won't match the name I have on form DS-3025 ( vaccination report) I had in Ukraine before obtaining an American visa?

I had an MMR, but a doctor in Ukraine only checked for me Rubella, and left out Mumps and Measles. Will they make me re-take vaccinations again? I am highly allergic to some types of medications and it really bothers me,,,,
