A certificate can be obtained by going to any of the National Statistics Office Serbilis Centers listed on this http://www.census.gov.ph/directory/census-serbilis-center-metro-manila webpage and filling out a request. The fee is 195 Philippine pesos, and processing of the request takes approximately 1 week, after which it can be picked up.

A certificate can also be obtained by filling out a request online at this https://www.ecensus.com.ph/Default.aspx site. The fee is 315 Philippine pesos, paid via this https://www.ecensus.com.ph/Secure/PaymentInstructions.aspx webpage, and processing of the request takes approximately 1 week, after which it will be mailed.
