Hello all!

Our interview date is coming closer and of course we are freaking out! I have a question as to what "Form DS-3026" is since I saw it on a few topis here on this website and that it needs to be filled out prior to the medical? We did not get that form or anythign when we got the email telling us the interview date and the required documents and I was under the impression that it gets filled out there on the day of the medical if it is a medical form????? Our interview is in Naples.

I am so lost! Oh and this is for a spousal visa by the way....

Also, if there is a vaccine he needs that takes 2 doses at separate times or something like that --> he has to go back??!?! This will cause more of a delay??? Or can he have the second dose done here in US if he was planning on being here immediately (assuming and hoping all goes well and we get approved).

Please help with some insight. We are almost at the end and I think we are in panic mode to think that anything else can delay us finally being together cray5ol.gif I want to make sure everything goes ok the day of the interview.

Hi Barbara!

We are also going through Naples (although for k1). Ds 3026 you only need to fill out part one, the past medical history part. And name and what not above. Here's the link: http://photos.state.gov/libraries/indonesia/39176/pdfs/IV_DS-3026.pdf

Just print it off and fill it out. It only takes a couple minutes :)

I'm not sure about the vaccinations but with k1 there is another exam after. I really don't think needing one later will delay anything, they will just expect you to get it in the U.S.

It should be with the medical forms as it's a forum the doctor normally does.

He shouldn't have any issues with the medical. None of the vaccinations required are two shot ordeals unless he's never had them before, but generally speaking they let them do the second set in the USA. (That I've seen with similar posts on VJ.)

Oh great! When is your interview? We are so nervous and honestly the email packet we got from NVC didn't tell us anything about the DS3026 form so I was really confused reading embassy reviews and people mentioning to bring that form. Thanks for the info and the link.

You mean they will have it there and he will need to fill out there? I guess I will download it from the link posted and maybe just have him take it anyway. As for the shots, thanks-I was really nervous about that!!

As far as I understand he doesn't fill it out.. the Dr does. I never had the DS-3026 form at all.

But let me look into it more and get back to you. :)

everything I can find DS-3026 is not a downloadable form, it will be something from your medical that the Dr will fill out and send in your do not open medical package.

When we had received our packet 3 from Naples, the ds 3026 had been included along with an attachment with instructions to fill in name, dob, and section 1 past medical history. I don't think filling out those parts would hurt at all, since they do not take more than a couple of minutes and then you will be sure you have everything when you go. You are going to be going in before July 15th? When is your interview date?

You are a K1 and this the CR1/IR1 forum so it's a bit different. We don't receive package 3.

yes it's a bit different for us but thanks anyway!!! And good luck on your interview! Ours is July 23 and we are freaking out (but in a good way) :D
